All of the JVI Forms are listed below. Click on the form name to download it and save it to your computer for printing or completing and emailing.   These are Victorian State forms only and should be submitted to the JVI.   In general, where payment is required, the form should be submitted to the treasurer.

For forms which require an actual signature and which cannot be signed electronically, should be printed, signed by all signatories and scanned.   It is acceptable if a copy of the form is emailed from each signatory’s personal email address along with an email confirming that the signatory approves the form.  As a last resort, they may be posted to the appropriate person at:

An additional 6 days should be allowed for postal delays.

We request that forms are emailed or mailed to the appropriate location as listed above. Forms will not be accepted in person at tournaments or events.
